Reid was in Chicago for a celebrity signing event when she retired to her hotel, the DoubleTree Chicago O'Hare Airport — Rosemont, on Nov. 23. She went down to the bar for a drink but was later taken out on a stretcher, police confirmed.
Video footage obtained byTMZshowed paramedics moving Reid from a hotel lobby to an ambulance via a stretcher. A spokesperson for Rosemont Public Safety later confirmed to PEOPLE that they were called to the scene for "a sick person" at 12:39 a.m. After taking her out of the hotel, she "was transported to the hospital," per the authorities.
In the video, Reid is seen struggling to speak and stand while onlookers assisted her in getting seated in a wheelchair. When a nearby man tried to help her, she appeared confused and was overheard incoherently saying, "You don't know who I am. I am famous. I'm an actress."
Shortly after the video surfaced, Reid's representative claimed that she had "filed a police report after an incident in which she believes her drink was tampered with." Police confirmed to PEOPLE at the time that they had not yet received a report from Reid.
"She is cooperating fully with the investigation," continued the rep statement. "Tara is recovering and asks for privacy during this traumatic time. She also urges everyone to be careful, watch your drinks and never leave them unattended, as this can happen to anyone. She will not be making further comments at this stage."
She filed a police report two days later claiming she was drugged
On Nov. 25, Reid filed an official police report with the Rosemont Police Department, in which she alleged that she "completely blacked out" after having one drink with a man at the hotel bar.
In the report, Reid recalled seeing "a bunch of people in the lobby that were supposedly YouTubers" that all had "their phones out and the whole thing felt suspicious but I didn't know what was going on."
TheUrban Legendactress claimed that after ordering a glass of white wine, "a man came up to me and introduced himself as Sean P, a very successful influencer and YouTuber." After chatting at the bar, the two went outside to smoke a cigarette and exchanged phone numbers, she claimed, adding that when she returned to the bar, she noticed that "there was a napkin over my drink."
"Then the last thing I remember was drinking that drink and seeing a couple sitting next to us," Reid alleged in the report. "The next thing I remember was waking up in the hospital around 8 hours or more later not knowing where I was or anything that happened. I completely blacked out with no recollection of anything."
Reid claimed that the YouTuber later "texted me the videos of me the next day" and alleged that she thought it was an extortion attempt. Meanwhile, he allegedly confirmed that Reid only had one drink while they were together.

After Reid filed her police report, the local police began investigating the incident. On Dec. 3, the Rosemont Public Safety Department told PEOPLE that they were unable to find evidence that Reid was drugged from the hotel's video footage.
"While video surveillance showed Tara Reid at the hotel bar, at no time did video show anyone tampering with or adding something to her drink," police said in a statement to PEOPLE. "We can confirm that a bartender covered her drink when she left the bar which is standard practice for bartenders to do."
The authorities explained that they are waiting on Reid's hospital records to proceed, but they had "not received" those records as of Dec. 3. They confirmed that she received "chemical testing" but could not elaborate on the "specific" tests.
"Pending those results or further information coming forward, our detectives will follow up on those leads accordingly," the statement continued. "At this time there is no criminal act that has been committed and there is no one we are currently investigating."
